Your First Visit

When you come for your appointment please remember to obtain and bring the following:

Driver’s License or valid ID
Insurance information
List of medications
Important: Please help Dr. Myer be able to give you the best treatment recommendations.
Previous tests: It is very important to bring the images on a CD (X-rays, MRIs, CT scans, etc.).
Previous surgery: Please contact previous surgeon/s for any operative reports / arthroscopic pictures. Please bring or have faxed to Dr. Myer’s office (330) 535-4415
